- Customer Information
Fill
in the top part. Name, Address, City, State and Zip. Be sure to
include a phone number in case
we have questions
concerning your print order.
2 - Shipping
Information
Fill in if your shipping
address is different than your home address.
3 - How to Ship your Order
Indicate how you want us to
ship your order. FedEx, UPS, Airborne, US Mail Service Priority Mail, or
Other carrier. In
addition, please indicate how fast to ship.
(Overnight, next day, 1day, 2
day, 3 day, etc.).

- Payment Method
Place
a check mark by your preferred method of payment. If paying by credit
card, download the
Credit Card Payment Form.
If you are paying by check, please take note that checks must clear the
bank before we can ship
your order to you.
5 - B&W Film
Process Order Form
# of Rolls
- Number of rolls enclosed of this type of film (i.e. TriX, PlusX etc.)
Film Size
- 35mm or 120 size roll
Type
- Name of the film. (TriX or TX, PlusX or PX, HP5, FP4, APX
(Agfa), NEO (Neopan),
IF
(Infrared) etc.)
Exposures
- Number of exposures on the roll (24 or 36 for 35mm), (12, 15, 16 for 120
size film)
ASA
- Put in the ASA of the film as you shot it. Place an N here for
Normal ASA if you shot
the film as it was rated.
ASA Change - (+)
This means to PUSH the film or increase the development time by however
many
stops you overexposed the film (such as: 1 stop, 2 stops, 10 %, 20%, etc.)
ASA
Change - (-)
This means to PULL the film or
decrease the development time by however many
stops or percentage you under exposed the film. (Such as: 1or 2 stops, 10%, 20%)
Machine Prints-Do you
want machine prints from this roll YES or NO.
Extra Proofs
-Do you want any extra proof sheets made on this roll. Place an X or Yes here if
you
do want extra proof sheets.
6 - Special
Instructions
Write here any
special instructions concerning the film processing you prefer here.

- Machine Prints
There are 2 choices for
size of machine prints 3½x5 and 4x6. Place an X in the box next to the
size prints you prefer.
Number of copies per
- How many copies of each frame do you prefer (1, 2, 3)
Paper Surface - Choose
which surface you prefer (Glossy, Pearl, Matt)
Borders -
W = WITH
Borders (A ¼" white border is a normal size border).
W/O = WITHOUT
Borders (Also called Borderless)
SBL = SLOPPY
black line (Sloppy Black borders will show some of the negative
edges.
All Sloppy Bordered prints will be Full Negative and cannot
be cropped.)
Special Instructions - Any
special instructions concerning your machine prints.
