Processing Order Form

Customer Information
Fill in the top part with Name, Address, City,
State and Zip code. Be sure to include your phone
number in case we need to confirm your order or
if we have questions concerning printing your order.

Shipping Information
Fill in this area if your shipping address if
different than your Customer Information.

How to Ship your order
Indicate how you want us to ship your order.
FedEx, UPS, Airborne, US Mail Service Priority Mail,
or Other carrier. In addition, please
indicate how fast to ship. (Overnight, next day, 1day, 2 day, 3 day).

Method
Place a check
mark by your preferred method of payment. If paying by credit card,
download the Credit
Card Payment Form. If you are paying by check,
please take note that checks must clear the bank
before we can ship your order to you.

Black & White Print Order Form
Film Roll #
- Place the number or name of the roll of negatives used for this
print here.
Neg Frame # -
Place the neg number that is located directly in the middle of the negative
frame you
want printed. If the number in not in the middle use both numbers within
the frame.
(Such as: 24-24A or 18A-19).
Print Copies
- How many prints do you need on this frame number.
Print Size
- What size print do you need on this frame number.
RC Surface
- You have a choice of Glossy, Pearl
or Matt. Place an X in the
appropriate box you
prefer (G,
P, M).
Fiber Surface - You have
a choice of Glossy or Matt
in either ILFORD MG Paper or Warmtone Paper.
Please place an X in the appropriate box G or M.
Borders
- W = WITH
Borders (A ¼" white border is a normal size border).
W/O = WITHOUT
Borders (Also called Borderless)
SBL = SLOPPY
black line (Sloppy Black borders will show some of the negative
perforations. All Sloppy Bordered prints will be Full Negative and cannot
be
cropped.)
TBL = Thin
Black Line (A thin black line will be placed around the image
and inside
the normal white border. Thin black line prints can be cropped.)
Full Neg
- Full Negative or Full Frame means the entire negative will be
printed. A 35mm
negative is 7" x 10", 8x12, 9½x14. etc.. In order to print the
regular size 8x10, 11x14,
16x20, it is necessary to "crop" the negative. A 35mm negative
is longer than most
print sizes. Therefore, we must exclude some of the negative from being printed.
Crop to Fit Size - Place an X here if
you want us to make a minimum crop to fit the size of the print
needed. If you have a special crop for this frame # please enclose a print with
the
specific crop you need indicated on the sample print to insure it will be
cropped
correctly.
